Georgia Alzheimer's and Related Dementia's (GARD) Task Force Survey

The Georgia Alzheimer's and Related Dementia's (GARD) Task Force requests your participation in a statewide survey regarding gaps in knowledge and need for expanded training in caring for persons with Alzheimer's and other dementias across our state.

Please note that the "Providers" survey targets primarily administrators, clinical managers, resident care services managers, and others who may oversee multiple care providers and/or facilities. The "Workforce" survey is for those who directly care for persons with Alzheimer's and other dementias.

Responses will remain anonymous and reported -in aggregate form- by Emory University to the GARD talk Force. We appreciate your support of this important endeavor!
